F-16 Crash and Pilot’s Death. Investigators Reveal the Direction of the Case

The prosecutor’s office has not decided to appoint a special investigative team regarding the F-16 crash in Radom; the case will be handled by two military prosecutors from Warsaw, said Piotr Antoni Skiba, spokesperson for the Warsaw District Prosecutor’s Office, on Thursday. It is now known in which direction the proceedings are moving.

He reported that the investigation is being conducted under the charge of unintentionally causing an aviation disaster resulting in the death of the pilot.

Prosecutor Skiba noted that, so far, a total of 48 witnesses have been questioned. He also said that the camera belonging to the pilot had been subjected to an initial expert analysis by a military police IT specialist.

“It is operational, and it contains recordings that have been secured. At a later stage of the investigation, they will be examined in great detail and compared with other evidence,” he emphasized.

He added that footage from the Krzesiny and Radom airbases has also been secured. “They are being analyzed for content,” he said.

The accident occurred last week during training flights for the AirSHOW in Radom. The F-16, piloted by Major Maciej “Slab” Krakowian, crashed; the pilot did not survive.

Born in 1988, Major Krakowian was an outstanding Polish military pilot, leader of the F-16 Tiger Demo Team, and an instructor at the 31st Tactical Air Base in Krzesiny.

A graduate of the Air Force High School in Dęblin and the United States Air Force Academy in Colorado Springs, he logged more than 1,400 flight hours, including around 1,200 on the F-16. Known for his dynamic air displays, in 2025 he received the prestigious As the Crow Flies Trophy at the Royal International Air Tattoo in the United Kingdom.

He was expected to be the star of the upcoming AirSHOW Radom 2025.
